Given to me by my brother-in-law about 20 years ago. I think he might have seen an ad in a magazine and sent away for it. Or maybe he got it done at a novelty shop in Calgary, where he lives. I don’t quite remember. Regardless, he thought a picture with my name on the Forum scoreboard was a good present.
He was right. It’s awesome.

This IS awesome. I do believe there is a video or sound clip you can get where it sounds like you’re the hero of the game. I’ll see if I can find it again.
I’d be thrilled with this as well!
